This is your first visit with the current Rehab staff for an injury (new or old) OR it has been more than 6 months since you've been seen by a member of the rehab staff. Subsequent visits after your initial evaluation should be scheduled as a "Follow-up Visit". *ONE BODYPART PER EVALUATION APPOINTMENT*
